Aspire Health: A holistic approach to palliative care

We’re committed to providing innovative ways to support your well-being — especially during life’s most difficult moments. That’s why we’re pleased to partner with Aspire Health, a palliative care provider that offers 24/7 clinical support, including access to a team of clinicians, for advanced stages of illness.

You may have already heard from or received information from Aspire Health. If you’re eligible, you will be invited to participate in this voluntary program through a referral to Aspire by a Harvard Pilgrim care manager, a primary care provider (PCP) or a specialist, or based on claims history.

How does Aspire Health work? 

Patients, caregivers and family can all have peace of mind, knowing they have the support they need to navigate complex medical decisions. This means:

Round-the-clock access to Aspire Health’s care team, including in-home, virtual and phone-based support

Help managing symptoms and access to a provider who can prescribe medications

Coordination of care among all doctors, in-home care and support services

Resources to address your questions about your illness, help you to make informed choices and ensure you understand the community resources available to you

Care planning with discussion about health care goals, preparing for the unexpected and assistance with advance directives

Fewer hospitalizations and emergency room visits, leading to lower out-of-pocket costs for members
