Fertility services

Many Harvard Pilgrim plans cover certain fertility services for eligible members. Covered services typically include:

  • Specialist consultation (PCP referral required for HMO members)
  • Diagnostic services medically necessary to assess fertility, such as:
    • Lab work
    • Hysterosalpingogram
    • Laparoscopy
    • Ultrasound
    • Sperm testing
  • Fertility support services and procedures, including:
    • Intrauterine insemination (IUI)
    • In vitro fertilization (IVF)
    • Frozen embryo transfer (FET)
    • Gamete/zygote intra-fallopian transfer (GIFT/ZIFT)
    • Microsurgical sperm aspiration (MESA)
    • Testicular sperm extraction (TESE)
