Program Overview

1. Not yet a Harvard Pilgrim member or don’t have your ID #?

If you’re new to Harvard Pilgrim but haven’t yet received your member ID information, please give us a call at (888) 888-4742, ext. #31209.

If you’re new to Harvard Pilgrim or haven’t yet received your member ID information, please give us a call at at (866) 874-0817. Our SmartStart team will be happy to talk with you about your new benefits and put you in touch with clinical experts to discuss your medical concerns. SmartStart is a dedicated call center for people thinking of joining Harvard Pilgrim.

2. Have your ID # but need to set up your online account? 

It’s easy to do. Visit harvardpilgrim.org and follow the simple instructions after the “Member Login” prompt.   

3. All set? Now, connect with your new care team!

Once you have your Harvard Pilgrim ID# and have set up your online member account, be sure to call our Care Management team at (866) 750-2068, ext. 33523.  We’ll contact you soon after to talk about your specific needs and get you lined up with the right care team member. This dedicated nurse will help you access and make full use of the Diabetes resources highlighted below.

Expert Guidance, Personalized Support

A dedicated care manager will work with you to develop a customized care plan and provide ongoing help with any diabetes concerns and services, so your care is well managed and you can make the most out of your new plan. As a member, you’ll also get: 

  • Timely touchpoints, including annual care reports and reminders for important tests
  • Access to a team of certified Lifestyle Management Coaches

Pharmacy Benefit

The following diabetes care supplies are covered* and available through a network pharmacy with your doctor’s prescription:  

  • Freestyle Test Strips by Abbott Diabetes Care
  • Freestyle Libre Flash Glucose Meter by Abbott Diabetes Care
  • Freestyle Libre Sensors by Abbott Diabetes Care
  • Urine/Ketone Strips
  • Lancets
  • Insulin
  • Syringes and Disposable Pens
  • Oral Diabetes Medications
  • Certain Freestyle Blood Glucose meters:  the FreeStyle Lite or FreeStyle Freedom Lite Blood Glucose meters are available at no cost and do not require prior authorization. With a prescription, members may order one of these meters online or through their pharmacy at no charge. (Please be aware that blood glucose meters not made by Abbott are not covered and other restrictions may apply**.)

*Copayment and/or deductible may apply depending on your policy.

**With the exception of the FreeStyle Lite or FreeStyle Freedom Lite Blood Glucose meters (see final bullet above), Prior Authorization is required for all other diabetes management devices including Intermittent/Professional Continuous Glucose Monitors, Long Term/Personal Glucose Monitors, Insulin Pumps and Sensor-Augmented Pump Therapy.
