ChoiceNet PPO
A PPO that assigns in-network providers to one of three tiers based on cost and quality performance. Member pays different cost-sharing based on a provider's assigned benefit tier. Providers' tiers are noted in the ChoiceNet provider directory and may change annually on January 1. Provides out-of-network access to non-participating providers. PCPs and referrals are required.
