A qualified high-deductible health plan (HDHP). Provides in-network access to local and national providers and hospitals, and out-of-network access to non-participating providers. PCPs are required; referrals are not. Gives employers and/or members the option to fund a health savings account.

POS Open Access - LP
Provides in-network access to local and national providers and hospitals, and out-of-network access to non-participating providers. PCPs are required; referrals are not. With the LP benefit, members can pay lower cost sharing when they use Select LP Providers for lab services and outpatient surgery.
