Accessing Care

As industry leaders in population health, we combine data and analytics with robust care-management outreach. Our “whole person” approach to care encourages wellness and contains costs. All members have access to health care professionals who are certified care managers and wellness coaches, licensed social and behavioral health workers, and nurse educators specializing in diabetes, pediatrics, asthma and cardiology.

Whether fully or self-insured, all our plans have industry-leading programs aimed at supporting members wherever they are on the continuum of health—at no additional cost. Our programs range from helping members living with conditions such as asthma, diabetes, heart disease and COPD—to those at risk for hospitalization due to multiple conditions and diagnoses—to those who could benefit from lifestyle support programs, such as pregnancy, smoking cessation, nutrition and weight management.

Access to a national network of 830,000 providers and more than 5,800 hospitals via UnitedHealthcare.

Our value-based provider contracts and deep partnerships drive better health and deliver greater value—members get the right care at the right time in the right place. Together, our team works with members’ primary care providers to put them on a path to better health.

As of July 2018, seventy-six percent of our HMO and POS members receive care from provider groups with value-based contracts

Adjusted medical costs trends are 3% lower for providers in value-based contracts than those who are not*.
