As we noted in last month’s issue of the newsletter, beginning Feb. 1, 2021, Harvard Pilgrim Health Care is partnering with Aspire Health to offer a new palliative care program to our commercial members who are facing an advanced illness.
Aspire’s team of doctors, nurse practitioners, nurses, and social workers provide an extra layer of support for members. The program assists members with complex medical, behavioral, social and palliative needs, aiming to prevent unnecessary hospitalizations, provide relief from symptoms, and offer support with managing the stress of having a serious illness.
Members who choose to participate receive visits either in-home or virtually. Harvard Pilgrim and Aspire will be reaching out to members who may benefit from the program in the coming months. Providers can also refer members into the program.
Aspire doesn’t replace the care of PCPs and specialists; members enrolled in this program keep their PCP and other specialists and may continue to seek curative treatment. The Aspire care team collaborates closely with the member’s PCP and specialists, sending a one-page report after each visit. These reports include a summary of the visit, a reconciled list of medications, and contact information for the Aspire lead physician and advanced care physician. In addition, the Aspire clinical team will consult with the member’s PCP and specialists regularly to discuss any significant changes to care plans or medications.
