As a reminder, it’s important to notify Harvard Pilgrim if your skilled nursing facility (SNF) will be undergoing any changes in ownership, management, or operation. Please send written notice to Harvard Pilgrim at least three months prior to any sale or transfer for changes in ownership or management of your skilled nursing facility or four months prior to a SNF closing. Because such changes affect our members’ access to care, provider agreements, and payment of claims, it’s important that Harvard Pilgrim receive appropriate notice so that we may review and plan for this transition.
When notifying Harvard Pilgrim of these types of changes, please include the following information:
- An overview of the change
- Effective date
- Legal and DBA name of the provider
- SNF’s tax identification number (TIN)
- SNF’s national provider identifier (NPI)
