Continuity and Coordination of Care

As part of on-going efforts to strengthen continuity between medical and behavioral health care, Harvard Pilgrim has established (and communicated to its behavioral health benefits manager) policies designed to improve the continuity and coordination of behavioral health care with care provided by PCPs and other medical providers.

  • The policy regarding Exchange of Information* describes Harvard Pilgrim's expectations regarding the exchange of information between behavioral health (BH) and medical practitioners, specialists, and providers, and the mechanisms in place to ensure the timely, effective, and confidential exchange of relevant information.
  • The policy regarding Diagnosis, Treatment, and Referral of Behavioral Health Disorders* describes the mechanisms in place for assessment, treatment, and referral of behavioral health disorders commonly seen in primary care settings.
  • The policy regarding the use of Psychopharmacological Medication* describes the mechanisms in place to evaluate the use of psychopharmacological medications.
  • The policy regarding Co-Existing Medical and Behavioral Health Disorders* describes the mechanisms in place for assessing processes for the screening and management of patients with co-existing medical and behavioral health disorders.
