Specialty Pharmacy Program Updates 


Harvard Pilgrim’s Specialty Pharmacy Program has added the following medications for both the Premium and Value formularies.

Name Indication Coverage Available From

Farydak (panobinostat)

Farydak is a histone deacetylase inhibitor that is used in combination with bortezomib (Velcade) and dexamethasone in the treatment of patients with multiple myeloma who have received at least 2 prior regimens. Pharmacy Benefit Accredo

Novoeight [antihemophilic factor (recombinant)]

Novoeight is an injectable medication that is used in adults and children with hemophilia A for:
  • Control and prevention of bleeding
  • Perioperative management
  • Routine prophylaxis to prevent or reduce the frequency of bleeding episodes

Natpara
(parathyroid hormone)

Natpara is used along with calcium and vitamin D to control hypocalcemia (low blood calcium levels) in patients with hypoparathyroidism. Pharmacy Benefit Accredo

Duopa (carbidopa/levodopa)

Duopa is an enteral suspension indicated for the treatment of motor fluctuations in patients with advanced Parkinson’s disease.  Pharmacy Benefit Accredo

Jadenu
(deferasirox) 

Jadenu is an iron chelator indicated for the treatment of chronic iron overload:
  • Due to blood transfusions in patients 2 years of age and older
  • In patients 10 years of age and older with non-transfusion-dependent thalassemia (NTDT) syndromes with a lower iron (Fe) concentration (LIC) or at least 5 mg Fe per gram of dry weight and a serum ferritin > 300mcg/L
