Diabetes Management Program

Program goals

Help our members control their diabetes and develop self-management skills, ensuring regular HbA1c and LDL screening, dilated eye exams, and screening for nephropathy.

Clinician Education and in-office supports

  • Case listings of patients with diabetes, highlighting diabetes tests that may be overdue and test results if available

  • Guidelines for effective care based on the American Diabetes Association Clinical Practice Recommendations

How to make a referral

Patients may be referred for diabetes nurse care management services by calling 1-866-750-2068.

Patients can be referred by their primary care physician, a nurse care manager, or may refer themselves.
