Asthma Disease Management Program

Program goals

Help members improve control of their asthma symptoms and reduce exacerbations through an increased understanding of asthma self-management.

  • Guidelines for effective care based on the National Heart, Lung, and Blood Institute recommendations

  • Provider tools as well as patient educational materials and information about resources available through our asthma nurse specialist

  • Patient case listings and utilization summaries

How to make a referral

Patients may be referred for asthma nurse care management services by calling 1-866-750-2068.

Patients can be referred by their primary care physician, a nurse care manager following an emergency room visit or hospitalization for asthma, or may refer themselves.
