Zostavax Available from Specialty Pharmacy

This is a reminder that Zostavax (zoster vaccine live) is available through Harvard Pilgrim’s specialty pharmacy program as an alternative to the traditional buy and bill method of distribution. Zostavax is an FDA-approved vaccine used in people age 50 and older to prevent shingles.

You may order Zostavax directly by calling or faxing a completed prescription form to Accredo Pharmacy. Prescription forms are available from Accredo upon request. Accredo will coordinate timely delivery, coinciding with your patient’s appointment for administration, ensure proper shipping, and bill Harvard Pilgrim directly with no up-front cost for you.

To submit a prescription for processing and dispensing, please contact the Accredo at:

Accredo Pharmacy
Phone: (866) 815-4717
Fax: (888) 773-7386

For more information, visit Harvard Pilgrim’s pharmacy page or call Clinical Pharmacy Services at (617) 509-1786.
