Harvard Pilgrim currently requires prior authorization for continuous glucose monitoring systems (CGMS), and will continue to do so. However, we are updating the coding associated with prior authorization requests for CGMS to better align it with current billing practices. Effective November 1, 2013, prior authorization will be required for the CGMS external transmitter (A9277) and external receiver (A9278). Please do not use code S1030 when submitting a request for CGMS authorization, as this code is no longer valid for DME services and using it will result in a rejection. For more detailed information, please review Harvard Pilgrim's CGMS Prior Authorization Medical Review Criteria, Diabetic Care Payment Policy, and Durable Medical Equipment Payment Policy.
