Harvard Pilgrim takes several steps to ensure well coordinated care for members treated at skilled nursing facilities (SNFs), surgical day care facilities, hospitals, and other ancillary providers, and when members transition between levels of care with a resulting change in practitioners. In order to strengthen communication and foster care coordination between SNFs and PCPs, Harvard Pilgrim:
- has a communication feature built into its credentialing/re-credentialing process
- conducts annual provider satisfaction surveys to evaluate the continuity and coordination of care between SNFs (and other ancillary providers) and PCPs
- contacts SNF administrators at facilities providing on-site care management in an effort to partner with them to improve member care
Harvard Pilgrim will again be sending reminder letters to SNF administrators and/or Directors of Nursing requesting their assistance in ensuring that timely communication occurs between SNFs and primary care providers whenever a member has an unscheduled transfer to an acute care facility, a planned discharge, or a transition to another level of care (or home), or has chosen to remain at the SNF for long-term care.
