Policy Update: Hizentra and Immune Globulin 

Effective immediately, Harvard Pilgrim has updated its Immune Globulin Prior Authorization Medical Review Criteria to include coverage for Hizentra (subcutaneous Ig) as an approved immune globulin medication that can be used as initial treatment for certain primary and secondary humoral immunodeficiencies, as well as for members who are unable to tolerate IVIG.

This change was made based on data showing Hizentra (J1559) to be effective in maintaining sustained levels of immune globulin over periods of time in the treatment of certain immunodeficiencies. As a reminder, prior authorization is required for immune globulin medication provided to Harvard Pilgrim's core HMO, PPO and POS members.
