How the Program Works

New Participants - Set Goals
After registering at the welcome desk, you will be screened for blood pressure, height and weight (body mass index), cholesterol, glucose and tobacco use. Based on your results, you and a clinical consultant will set attainable goals that focus on achieving or maintaining good health for each area.

At the next session, the Healthy Returns program will return to your campus to measure your progress and provide any additional support you may need to achieve your goals.

Returning Participants - Measure Your Progress
Based on the goals you set at your last session, the Healthy Returns program will measure your progress and provide any additional support you may need to achieve your goals.

Please Note: We understand the confidential nature of the information you are providing. Protecting your health information is paramount. Your health information will not be shared with USNH and your right to privacy will be protected according to Harvard Pilgrim privacy guidelines.

Get Rewarded!
Healthy Returns is a two-part program:

  1. Set goals and receive a $25 gift card
  2. Measure your progress at your next consecutive visit (in 6 months) and you are eligible to receive one of the following:
  • $10 Visa gift card for participating
  • $25 Visa gift card for participating and reaching one goal
  • $50 Visa gift card for participating and meeting two or more goals

Whether you're new to the program or have participated in the past, you'll receive a reward!

Please note: Gift cards awarded to USNH faculty and staff are considered income and are subject to withholding, regardless of amount.
