How to get care from out-of-network providers.
Under limited circumstances, our plan will allow our StrideSM (HMO) members to see out-of-network providers. These circumstances include seeing a provider with a specialty not currently contracted with our plan or seeing a provider while you are traveling outside our service area. StrideSM (HMO) has contracted with providers across our service area to ensure access to care for our members. You must get a referral from your network provider and receive prior authorization from the plan prior to your out of network Physician specialist visit. Your cost for seeing an out-of-network provider may be more.

If you need medical care that Medicare requires our plan to cover and the providers in our network cannot provide this care that Medicare requires our plan to cover and the providers in our network cannot provide this care, you can get this care from an out-of-network provider. However, authorization should be obtained from the Plan prior to seeking care. In this situation, you will pay the same as you would pay if you got the care from a network provider. You, your doctor, or your representative may call, write or fax our plan to make a request for authorization. For details on how to contact us, go to Chapter 2 in the Evidence of Coverage, Section 1 and look for the section called, “How to contact us when you are asking for a coverage decision about your medical care”.
