With a wide variety of enjoyable and healthy activities, Vitality will work with you to help establish and achieve your wellness goals. Reasonable Alternative Standard
Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees enrolled in a medical plan through John Hancock.
If you think you might be unable to meet a standard for a reward (reward can include avoidance of a surcharge) under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Vitality's Personal Pathways have been designed to provide all members with a Reasonable Alternative Standard and member-specific alternatives, which will be provided by Vitality, as advised by your healthcare professional to earn health-contingent points.
If you feel that you are medically unable to satisfy the requirements or the provided Reasonable Alternative Standard needed to earn points, please download and complete the medical waiver form and have it signed by a medical physician. You can earn Wellness Credits to reduce the cost of your portion of medical plan premiums by reaching Silver or Gold Vitality Status or higher by December 31, 2016. You must have medical coverage through John Hancock for the plan year in order for the Wellness Credits to apply. Learn more.
