  • This plan has significant annual deductibles. In-network is $1,300 per individual and $2,600 per family. Out-of-network is $2,500 per individual and $5,000 per family.

  • You can put money aside in a Health Savings Account to help offset your medical costs. Through premium, John Hancock contributes $500 for individual coverage and $1,000 for family coverage to this account as your employer.

  • PPO plans offer extensive freedom of choice - receive care from virtually any doctor or hospital without having to get a referral and you do not need to select a PCP.

  • You have "in-network" benefits when you receive care from providers who participate, and "out-of-net-work" benefits when you receive care from those who do not.

  • Most services, including prescription drugs,are subject to the deductible, then coinsurance. Select preventive care services are covered at no charge.
